Required Documents to get a Visa.
 
Valid passport (minimum validity: 6 months). Passport must have at least two blank VISA pages. Note: the "Amendment Pages" are NOT blank pages and visas cannot be stamped on them.  
One individual photo, PASSPORT SIZE (2X2") AND TYPE ONLY, front view, uncovered.
One application form duly filled in and signed (Signature must be original, no faxed forms).
Photocopy of round-trip ticket, itinerary or letter from travel agency/airline, confirming purchase of round-trip ticket, including dates of departure and return (for airline employees photocopy of airline ID card).
For applicants under 18 years of age, when traveling without parents or legal guardian, notarized letter of consent signed by parents or legal guardian (proof required).
For children 3 months to 6 years of age, certificate of vaccination against polio, stating type and dosage
